Start preparing your Lasting Power of Attorney and Advance Care Plan

Prepare for unexpected situations where you are unable to make decisions for yourself

What would you like to start with?

While you may choose to start with either, you are encouraged to complete both your Lasting Power of Attorney (LPA) and Advance Care Plan (ACP).

You are also encouraged to choose the same persons to:

  • make personal welfare decisions in your LPA

  • be your spokesperson in your ACP

This helps to reduce conflict in making healthcare decisions on your behalf.

Lasting Power of Attorney (LPA)

An LPA is a legal document that allows you to appoint one or more trusted persons to make decisions on your behalf if you lose mental capacity.

You can give them powers to decide on your:

  • Personal welfare: This includes healthcare and medical treatment decisions, day-to-day life decisions, and where you should live.
  • Property and affairs: This includes financial decisions such as managing your bank account, property, and CPF monies.

Advance Care Planning (ACP)

ACP refers to the process of planning for your future healthcare, for situations where you no longer have the capacity to make decisions or speak for yourself.

It includes:

  • reflecting on your personal beliefs and healthcare preferences
  • discussing these preferences with your loved ones
  • appointing a trusted person to speak on your behalf
  • recording your wishes in an ACP
